Territorial Admissions Committee
The Department of Health and Social Services’ Territorial Admissions Committee reviews applications for Long Term Care and Supported Living placement in facilities that are funded by the Government of the Northwest Territories. The Territorial Admissions Committee provides a unified process for admission to Long Term Care and Supported Living, and ensures fair and equal access for residents across the NWT.
The Long Term Care and Supported Living application includes a comprehensive nursing, medical, and functional assessment of the person’s care needs to determine eligibility. Once the applicant is found eligible for admission, his or her name is placed on a Territorial Wait List for a Long Term Care or Supported Living bed.
If you are interested in applying, contact your Regional Health and Social Services Authority Home Care Program or Community Health Centre for help with the application.
