Reinstate Your Medical Licence
What you need to provide
- Completed Application for Medical Licence with a recent passport type photo of you attached.
- Specified location of work (please specify name of clinic or hospital) and planned start date.
- Certificate of Standing from all licensing authorities where you are and have been licenced.
Note: Licensing authority must mail Certificate of Standing directly to the NWT Professional Licensing Registrar.
- Up-to-date and detailed curriculum vitae/resume.
- Three references, from individuals who can comment on your work, sent directly to the Registrar. Reference form can be found in the application package. Referees must have worked with the applicant within the past three years.
Note: The Registrar cannot provide anyone with copies. If applying for hospital/health board privileges, have separate references sent directly to them.
- Photocopy of proof of citizenship i.e. Canadian birth certificate, or valid immigration document.
- Credit card authorization or cheque or money order made payable to Government of Northwest Territories.
Failure to ensure all documents are forwarded to the Registrar’s Office as stated above will delay and/or possibly prevent licensing.
** GENERALLY, PLEASE ALLOW 6 WEEKS FOR PROCESSING OF APPLICATIONS **
Application Package requirements posted on the Professional Licensing website supersedes any prior application packages in circulation

Registration and annual licence: $323.00 ($108 initial registration fee plus $215 due annually) - licence expires March 31st following date of issue, renewable upon payment of fee
Registration and limited permit: $162.00 ($108 initial registration fee plus $54.00 for each subsequent permit) valid for 3 months from the date of issue (not renewable, may be applied for twice in a fiscal year)
Research permit: valid for 12 months maximum - $108.00 (licence can be renewed upon application)
