Renew Your Midwifery Registration

What you need to provide

  • Completed NWT Licence Renewal Application Form  before December 1st along with the following supporting documents:

    • Proof of completion of a course in adult and infant cardiopulmonary resuscitation (Basic Life Support) that complies with the guidelines of the Heart and Stroke Foundation of Canada, not more than one year prior to the date of your application for renewal.

    • Proof of completion of a course in neonatal resuscitation, including intubation, not more than one year prior to the date of application for renewal.

    • Proof of completion of a course in the Fundamentals of Fetal Health Surveillance, not more than two years prior to the date of application for renewal.

    • Proof of completion of a course in Emergency Obstetrical Skills, not more than two years prior to the date of application for renewal.

  • A Practice Audit is required every three years. The Office of the Registrar will notify registrants by way of email at renewal time (mid October)  to provide proof of successful completion of the Practice Audit.
  • Credit card authorization or cheque or money order made payable to Government of Northwest Territories.

Note: Registrar, Professional Licensing will email NWT Licence Renewal Form in October. Licence and other correspondence will be sent to mailing address unless advised otherwise.

Failure to ensure all documents are forwarded to the Registrar’s Office as stated above will delay and possibly prevent licensing.

All active registrants will receive renewal notification mid-October and the renewal application from the Office of the Registrar, Professional Licensing, Department of Health and Social Services.

Reinstatement (late renewal) $161

Note: All licences expire March 31st following date of issue, renewable upon payment of annual fee.
