CCIS Hub Overview
What is CCIS?
The CCIS (Continuing Care Information System) is the new territorial digital platform designed to support Home and Community Care (HCC) and Long-Term Care (LTC) services. The Northwest Territories will be using the CCIS for:
- Tracking service delivery for Nursing and Personal Support Worker (PSW)/Home Support Worker (HSW);
- Reporting Home Care Median Wait Time to the Canadian Institute for Health Information (CIHI); and
- HCC and LTC interRAI Assessments
How to Onboard to CCIS
The CCIS Hub is the central location for on-boarding resources. Simply use the menu on the left side of your screen to navigate through the onboarding steps.
