Who can request information under the NWT Child and Family Services Act?
- Any person that is currently involved with or has been historically involved with the NWT Child and Family Services Program.
- Any person that has the lawful custody of a child or youth that is currently or has been historically involved with the Child and Family Services Program.
What two (2) pieces of identification will I need to provide?
Please provide (1) copy of Government issued identification with a photo identifying yourself and one (1) copy of a piece of personal identification that verifies your current mailing address.
How do I obtain information that has been collected under NWT Child and Family Services Act?
Please complete, sign and submit the form Request for Information Collected under the Child and Family Services Act along with two (2) pieces of identification..
What information is required to provide proof of lawful custody of a child or youth?
Please provide a copy of a valid court order, separation agreement, guardianship order and or consent of another person who has lawful custody of the child or youth that demonstrates that you are authorized to receive the information requested. A youth 16 – 18 years of age, must also provide their consent if they are capable of providing informed consent for their information to be shared with a person who has lawful custody of them.
How long will it take to receive the information that I requested?
Please allow between 7 – 30 days to process your request. Depending on the volume and or the nature of any specific information that was requested, the Records Management Coordinator will contact you and you will be informed of a new timeframe if the search and release of information will take longer than 30 days.
