Who can order a birth certificate?
- the person named on the birth registration
- individuals recorded as parents on the birth registration
- a legal representative (e.g. guardian, solicitor or legal agent) of the person whose name appears on the birth registration
- a child or grandchild of the person whose birth is registered.
- an agent, or any other person, on the written authorization of the person concerned or the parents
- a person who requires the certificate for use in a court of law, for adoption or for settlement of an estate
- an officer of the Crown for official purposes
- government departments approved by the Director of Vital Statistics, when required for legal or other legitimate purposes.
To order a birth certificate:
- Complete and submit the Application for Birth Certificate form:
- online, using eServices;
- email to healthcarecard@gov.nt.ca;
- fax (if paying by credit card only): 1-867-777-3197

Prerequisites
- Certificate can only be issued for births that occurred in the Northwest Territories.

Available online or you can request a copy from the Registrar General of Vital Statistics, Health Services Administration Office.
Fees
- Birth certificates are $22.00. $33.00 for a Restricted Photocopy.
- A reason for requesting a Restricted Photocopy must be provided, and it will only be released at the discretion of the Registrar General of Vital Statistics.
- Pay by credit card, cheque or money order (payable to Government of the NWT).
