Who can order a death certificate?
- member of the immediate family or next of kin
- a person who requires the certificate for use in a court of law, for settlement of an estate or for disinterment purposes
- an officer of the Crown, for official purposes
- government departments approved by the Director of Vital Statistics, when required for legal or other legitimate purposes.
To order a death certificate:
- Complete and submit the Application for Death Certificate form:
- online, using eServices;
- email to healthcarecard@gov.nt.ca;
- fax (if paying by credit card only): 1-867-777-3197
- mail to:

Prerequisites
- Certificate can only be issued for deaths that occurred in the Northwest Territories.

Fees
- Certificates are $22.00. Only paper (framing) size available.
- Pay by cheque or money order (payable to Government of the NWT)
