Is your Health Care Card Expiring?

Check the bottom right hand corner of your health care card for expiry date.

You are responsible for renewing your health care card. Renewal can be completed four months in advance of the expiry date. 

You can download the form NWT Health Care Card Renewal or call Health Services Administration at 1-800-661-0830. The form can also be picked up at your nearest health centre or hospital.

Did you lose or damage your NWT health care card?

If you lose or damage your NWT Health Care Card, please contact the Health Services Administration Office and request a new card immediately. You do not need to fill out a form. Once your identity is verified, the Health Services Administration Office will send you a new card in the mail.
