Who can apply for Extended Health Benefits (EHB)?
- A permanent resident of the Northwest Territories (NWT)
- Non-Indigenous
- Have a valid NWT Health Care Card
- For the Specified Disease Conditions Program, you must be under the care of a physician or nurse practitioner for one of these disease conditions:
*Indicates Specified Disease Conditions that are eligible for drug benefit only
Alcohol Dependency** Cystic Fibrosis Phenylketonuria (PKU)* Alzheimer’s Disease Dermatomyositis Primary Pulmonary Hypertension Asthma Diabetes Insipidus Psoriasis Bilateral Congenital Microphthalmos Budd-Chiari Syndrome
Diabetes Mellitus Rheumatic Fever Cancer

How to apply for the Specified Disease Conditions Program?
- A Physician or Nurse Practioner must complete and submit the EHB Specified Disease Conditions Program Application.
Please Note: If you have access to benefits under an employer or similar plan, and choose not to use them, you will not be eligible for assistance under the EHB Specified Disease Conditions Program.
A health care card is required to receive health care benefits.

Available at hospitals, health clinics, health centres and the Health Services Administration Office.
Fees
- There is no fee to apply for NWT health care.
