Applying for the Extended Health Benefits for Specified Disease Conditions Program

Who can apply for Extended Health Benefits (EHB)?

  • A permanent resident of the Northwest Territories (NWT)
  • Non-Indigenous
  • Have a valid NWT Health Care Card
  • For the Specified Disease Conditions Program, you must be under the care of a physician or nurse practitioner for one of these disease conditions:
    Alcohol Dependency** Cystic Fibrosis Phenylketonuria (PKU)*
    Alzheimer’s Disease Dermatomyositis Primary Pulmonary Hypertension
    Asthma Diabetes Insipidus Psoriasis

    Bilateral Congenital Microphthalmos Budd-Chiari Syndrome 

    Diabetes Mellitus Rheumatic Fever

    Cancer

    Drug Dependency** Rheumatoid Arthritis
    Celiac Disease* Epilepsy* Rickets*
    Cerebral Palsy Gastroparesis Scleroderma
    Certain disorders of the blood and immune system Head Injury Scoliosis
    Charcot-Marie tooth Hemophilia Sickle Cell Anemia
    Chronic Obstructive Lung Disease Hepatitis Spina Bifida
    Chronic Pancreatitis HIV Infection and other HIV related diseases Spinal Cord Injury - Paraplegia and Quadriplegia
    Chronic Psychosis Hypertension (subject to certain BP levels)* Tuberculosis
    Cirrhosis of the liver Ischemic Heart Disease Ulcerative Colitis
    Cleft Lip & Cleft Palate Lupus Erythematosus Wegener’s Granulomatosis
    Congenital Cytomegalovirus Infection Multiple Sclerosis  
    Congenital Anomalies of the Urinary System Muscular Dystrophy  
    Congenital Heart Disease Osteoarthritis*  
    Crohn’s Disease Pernicious Anemia*  
    *Indicates Specified Disease Conditions that are eligible for drug benefit only
    ** Indicates restricted benefits

How to apply for the Specified Disease Conditions Program?

Please Note: If you have access to benefits under an employer or similar plan, and choose not to use them, you will not be eligible for assistance under the EHB Specified Disease Conditions Program.


A health care card is required to receive health care benefits.
