Who can apply for Extended Health Benefits (EHB) Reassessment?
- Existing EHB clients whose:
- income has decreased 10% or more; or
- family unit has changed (such as marriage, separation, divorce, adoption, etc.)
How to apply for EHB Reassessment?
Complete and submit the Extended Health Benefits Reassessment Application form:
- email to healthcarecard@gov.nt.ca;
- fax to 1-867-777-3197; or
- mail to:
