Eligibility
To be eligible for changing your sex designation on your NWT birth certificate, your birth must be registered in the NWT.
How to apply for a Change in Sex Designation
19 Years of Age or Older
- Complete the Change of Sex Designation 19 Years of Age or Older form.
- Return all previously issued birth certificates.
- Complete an Application for Certificate of Birth/ Marriage / Death.
- Include payment for the change to your birth registration ($33) and a new NWT birth certificate ($22).
Under 19 years of age
- Your parent or legal guardian must complete the Change of Sex Designation Under 19 Years of Age form.
- Return all previously issued birth certificates.
- Complete an Application for Certificate of Birth/ Marriage / Death.
- Include payment for the change to your birth registration ($33) and a new NWT birth certificate ($22).
Between 16 and 18 years of age and living independently from parents
- Complete the Change of Sex Designation 16- 18 years of age, Independent Minor form.
- Return all previously issued birth certificates.
- Complete an Application for Certificate of Birth/Marriage/Death.
- Include payment for the change of your birth registration ($33) and a new NWT birth certificate ($22).
Prerequisites
- Birth must have occurred in the Northwest Territories.

Forms are available online or you can request a copy from the Registrar General of Vital Statistics, Health Services Administration Office.

If you also wish to change your gender marker in other identity documents, you will need to contact those agencies yourself:
- Driver’s License or General Identity Card (GNWT Department of Infrastructure)
- Passport (Government of Canada)
- Social Insurance Number (Government of Canada)
Please be aware that these agencies may not accept gender X or a single name. The Department of Health and Social Services does not have authority over other organizations as each will have their own requirements to access programs and services they offer.
